I had a similar experience. multiple Fluid air exchange was done to trace down residual oil PFCL mixture which was very difficult and time consuming to remove thru the 23 G system

Yes Dr. Medhat there was an unexplained poor BCVA after removal of the PFC/Oil mixture. My only explanation was that some sort of retinal toxicity had occurred.

I was wondering if you met any signs of retinal toxicity from the PFCL, being there in contact with the retinal surface for 3 months?

5000 cs is much more adherent to the PFO compared to 1000 cs. PFO/Si Oil interface is pinched where they are trying to separate and this is where you should approach the PFO to remove it first, as you suggest.

A Saline rinse following PFO removal can assist in removing any additional PFO bubbles.

had a same experience and i think there is bonding at the silicone oil pfcl interface which does not allow oil to float up in my case as soon as i removed pfcl bubble the residual silicone oil just floated up.
