Zeynep Aktas, MD, FEBO, presents a case of a 14-year-old male patient with refractory glaucoma in both eyes. He has a scleral thinning of unknown etiology. IOP was 20 mm Hg in the fellow eye with brinzolamide/brimonidine tartrate (SIMBRINZA, Novartis) fixed combination, and VA was 20/200. However, IOP was 45 mm Hg in the right eye with maximum tolerated medical therapy, and VA was 20/20. Prolene gonioscopy-assisted transluminal trabeculotomy (GATT) surgery is performed here in the right eye, and challenges encountered during GATT surgery in juvenile cases are emphasized.
