Microspherophakia is a rare congenital lens abnormality characterized by a small, spherical crystalline lens with increased anteroposterior thickness and zonular laxity. This video demonstrates extracapsular lensectomy with toric IOL implantation in the eye of a 25-year-old woman with autism spectrum disorder and global developmental delay who experienced significant visual distortion with spectacle-correction of pathologic myopic astigmatism. Preoperatively, the patient’s refraction measured -22.75+8.00×92 in the right eye and -22.75+8.00×89 in the left eye. The video highlights the surgical approach, including extracapsular lensectomy in the presence of a redundant capsular bag and placement of a capsular tension ring and toric IOL. Following surgery, residual refractive error was significantly reduced to -2.75+3.75×157 in the right eye and -3.50+2.00×41 in the left eye.
