Lisa Brothers Arbisser, MD, shares a case in which a 70-year-old woman presents for cataract surgery 10 years after sustaining an orbital fracture when she was kicked in the face by a horse. The injury left her with mild enophthalmos, progressive vision loss, traumatic glaucoma, and angle recession for 30º. Iridoplegia, pupil sphincter ruptures, a dense subluxated cataract, 5 hours of zonulolysis, vitreous herniation, and a ruptured anterior vitreous membrane are evident at the slit lamp.
