This patient underwent radial keratotomy (RK) 25 years ago. Several years ago, Dr. Siepser performed a photorefractive keratectomy for the patient's hyperopia. Recently, the patient became more hyperopic and an advancing cataract was noted. Care must be taken with these patients. Incisions can create the "pizza pie" event which is a splitting of the RK incisions and subsequent complications.
