A patient presented with a severely dislocated single piece acrylic lens. The sliding internal knot (SLIK) technique was attempted but the decision was made to explant the IOL. One haptic is externalized and the optic is grasped and folded with forceps in the wound and the entire lens-bag complex is removed. A 3-piece IOL is then implanted with a modified scleral glued IOL technique.
