Ozana Moraru, MD, shares a complex case involving angle-closure glaucoma with prior shunt implantation that has led to leakage. Several attempts to reform and preserve the anterior chamber failed, despite device removal and a scleral patch sutured on the leakage area. The eye developed bullous keratopathy and a hypermature white cataract. Dr. Moraru performed combined cataract extraction, endothelial transplant, and ultra-thin DSAEK (UT-DSAEK). The challenges in this case were related not only to poor visualization due to diffuse corneal edema, but also to the extremely flat anterior chamber, peripheral synechia, and the intumescent cataract.
