After the epithelium is marked, Dr. Milne uses an Epi-K epikeratome (Moria, Antony, France) with a 7.5mm suction ring displaced nasally for a myopic treatment (a 8.0mm stop is used for hyperopic treatments). Next, he places drops of preservative-free gel on the eye and on the separator to ensure that the epithelial surface is well lubricated. Once the separator has completely cleared the epithelium, he turns off the suction and lifts the device's head from the nasal to the temporal side, in the opposite direction of the epithelial sheet, in order not to displace it. After Bowman's layer is dried with a Wexel sponge and blunt spatula sweep, custom ablation is performed with a VisX S4 laser (AMO, Santa Clara, CA) and the flap is repositioned on the stromal surface.
