Orienting the IOFB with its shortest diameter / width would have prevented a large scleral opening. I did not see any reason for placing a buckle and doing a 360 laser, it could have been either, or neither (my preference). Chandelier illumination would have helped to scleral depress and search for iatrogenic breaks and only those could be lasered. I did not see that in your video.
