In intumescent cataracts and silicone–oil-filled eyes, the initial puncture can extend radially and rapidly in opposite directions, resulting in the Argentinian flag sign. The tear extends to the capsular periphery and, rarely, the posterior capsule.

1% sodium hyaluronate isn't appropiate for these cases. Cannot flatten the anterior surface of the lens
