Capsule Cleansing and Primary PCCC

Capsule cleansing is first performed to include the central posterior area with a silicone tipped cannula on a 5cc saline filled syringe and the undersurface of the anterior capsule using both a Singer Sweep (Epsilon) for the subincisional area and a Rentsch Capsule Curette (Geuder) for the remainder of the anterior capsule. A needle is then used to incise the central posterior capsule and OVD is placed into Berger's Space. A primary posterior continuous curvalinear capsulorhexis is then fashioned using forceps. A standard IOL is then placed into the bag and the case concluded.

its a wonderful technique.there is no need to polish anterior capsule if you trap optics of lens in PCC and since optical axis is going to be clear.i use this technique in patients below 50 years of age and mostly with three peace acrylic lens

I routinely perform capsule polishing of the posterior and anterior capsule; the cases that need it the most are the most difficult to clean: very loose zonules. Certain patients benefit from primary PCCC vs postop YAG: anyone who has difficulty with a YAG (cooperation, postioning, travel, etc) as well as patients you desire to have the best vision immediately postop. A YAG still is an effective treatment, but don't you find these capsules stubborn at times to YAG? Pitting the lens? Patients wondering why did you leave something in their eye?

what was indication for pccc ?

posterior capsular plaque from PSC cataract is the indication as stated in the voiceover; look closely as the pccc is torn and you can note adherent material being removed with the posterior capsulotomy

Just curious why not do a YAG in the post op period instead if the plaque is in the visual axis? Do you perform anterior capsule polishing on every case? Your video quality is great, and I appreciate seeing your techniques.
