Mun W. Lee presents a bimanual transconjunctival sutureless vitrectomy technique with a fourth port for 25G chandelier illumination. The author presents the use of this technique with ultra high speed cutting in several complicated retina surgery cases, including wolf-jaw detachment, rhegmatogenous retinal detachment, and "hammer and nail" penetrating trauma with IOFB. As the author demonstrates, this approach is particularly useful for membrane delamination in diabetic eyes with extensive fibrovascular proliferation, multiple combination of instruments, and application in PVR management or other complicated vitrectomies.
