Attaching Ahmed Glaucoma Valve to the Sclera

Ahmed Samir, MD, describes a technique to fix the plate of an Ahmed Valve plate using a flanged 6-0 prolene suture. The plate is fixed at 8 to 10 mm behind the limbus and the needle of the prolene suture is passed through the Ahmed Valve plate lateral to the tube insertion. A handheld cautery is used to create a flange at the free end of the suture. The needle is then passed through the sclera and back through the valve. The valve is inserted under the conjunctiva and maximum traction is exerted over the suture before creating the second flange.
