A 61-year-old diabetic patient with a history of multiple ocular surgeries presents with inferior graft melt, haptic exposure, flat anterior chamber, and IOL adhering to the cornea. Dr. Amar Agarwal's surgical plan includes penetrating keratoplasty, glued IOL implantation and Ahmed Valve procedure. After placing the Ahmed Valve, half of the corneal graft is excised while the fibrovascular membrane is removed. OVD-assisted and manual peripheral synechiolysis is performed and a posterior capsulotomy is performed with a blade. A three-piece IOL is implanted using the glued IOL technique. After removing necrotic corneal tissue, a new donor button is sutured into place and the scleral flaps are resposited with fibrin glue.

This is a great technique but I think it shold be standarized as a intraescleral IOL fixation, and not glued IOL

good lord! is there anything dr. agarwal cannot do? fearless and so skilled. being dr. agarwal's student ought to be mandatory for every surgeon.
