Despite DMEK's growing popularity, still, there are categories of eyes that are occasionally considered poor candidates for this operation. In particular, aphakic, post­vitrectomy, and eyes with unstable lens diaphragms have been commonly dismissed as ineligible. This video shows a stepwise approach to successful DMEK surgery combined with replacing an anterior chamber IO through a posterior iris­claw (Verisyse) IOL in the same procedure.
