A 17-year-old female patient presented with long standing vision loss in her left eye and a history of chronic retinal detachment. Vitrectomy was planned as causative retinal break was not found. A series of complications were faced, including choroidal exposure during scleral tunnels, subretinal migration of infusion cannula, intraoperative hemorrhage, extrusion of infusion cannula, epithelial edema and lens touch. Each complication was managed with corrective steps and the retina was successfully reattached.
