Dante Luis Buonsanti, MD, presents a case of a patient with a previous vitrectomy and a small pupil who had undergone cataract surgery 3 years ago, with a toric single-piece acrylic IOL placed in the sulcus. This case demonstrates the levitation of a subluxated IOL, cut in pieces and removed through a 3-mm incision. Later, a three-piece IOL was placed in sulcus, and, as there was abscence of a superior anterior capsule rim, one haptic was sutured to the iris.
