Drs. Schaal and Sigford present the treatment of an epiretinal membrane with 25-gauge pars plana vitrectomy. A 64-year-old woman presented with decreased visual acuity and metamorphopsia. Her best-corrected visual acuity measured 20/50. The surgeon injected preservative-free triamcinolone acetonide to assist with visualization of the posterior hyaloid face. Next, he instilled indocyanine green dye. He allowed the indocyanine green to sit against the internal limiting membrane and epiretinal membrane for 60 seconds, after which he removed it completely using a soft-tip cannula. He then initiated an ILM and ERM peel with a Tano brush. The surgeon completed the maculorrhexis with Eckhardt's forceps and then performed air/fluid exchange. Postoperatively, the patient's vision improved to 20/30, and the metamorphopsia resolved.
