Jeffrey Liegner, MD demonstrates the injection of TriMoxiVanc showing the plume of visible triamcinolone suspension (with two antibiotics) within the vitreous. Notice that viscoelastic is beginning to exit the wound as the IOL moves more anteriorly during expansion of the posterior segment, a helpful confirmatory sign. This intravitreal plume of medication is seen from the microscope's view in 90% of the injection events; 10% the medicine is properly placed but not visualized centrally. An endoscopic view of cannula's tip traversing the zonules during TriMoxiVanc injection with a 27 guage Knolle cannula (2mm distal segment) after IOL placement and under viscoelastic is shown. I makes me feel better that the zonules are not being torn but just pushed aside.
