You can use the yag to cut the vitreous wick.then do needling to make the bleb

i recently had a case of presumed suprachoroidal hemorrhage/effusion during trab. when i made the iridectomy, there was quite a bit of vitreous loss coming through the sclerotomy. i used weck cell to cut vitrous, but it continued to come out. so i then closed the scleral flap and the conj. the next day the eye was miraculously quiet with good bleb, deep ac and iop of about 10. i couldnt believe my luck. but after several weeks, the iop climbed to 26, and the bleb was turning into tenon's cyst. i could see, with gonio, sheet of vitreous going into the trab site. patient is 90 years old and in poor health, and i doubt she can go back for vitrectomy and revision of trab/tube.
