Sturge Weber Syndrome

Sturge Weber Syndrome (Encephalotrigeminal Angiomatosis) is a Phakomatosis. Glaucoma is reported in 30-71% of patients and among them 60% are congenital cases which respond poorly to medical management. Glaucoma surgery is associated with greater risk of complications like suprachoroidal hemorrhage and choroidal detachment. These occur secondary to rapid movement of fluid from intravascular to extravascular space due to increased episcleral venous pressure and sudden hypotony. In this video we demonstrate successful trabeculectomy in a 3-year-old female patient with left sided port wine stain and advanced glaucoma in left eye. Precautionary measures like sclerotomy, paracentesis, anterior chamber maintainer, preplacement of scleral flap sutures and tight tying of flap sutures were taken. Following which postoperatively, the patient had a diffuse bleb with good filtration, deep anterior chamber, no choroidal detachment and controlled IOP.
