Some surgeons, including myself, perform a trabeculotomy in conjunction with viscodilation during ab interno canaloplasty for many patients. For the original iTrack system, this would involve using a micro instrument to grab onto the distal end of the microcatheter, and then either pull this distal end to perform the trabeculotomy, or pull the catheter from the proximal end while continuing to hold the distal end steady.
With the new iTrack Advance system, there is an injector being held, which does not leave a hand available to grab the distal end of the microcatheter. This video describes a slick technique to perform the trabeculotomy using the injector.
