Mahmoud A. Khaimi, MD, presents a case of an obstruction in Schlemm canal during the catheterization process. Viscodilation is performed and OVD is pushed 6 clock hours distal to the obstruction, illustrated by blanching of the episcleral vessels beyond the obstruction site. A second case further demonstrates pressurized viscodilation. When blue stained OVD was injected inside Schlemm canal, it could also be visualized up in the anterior chamber, denoting that OVD works on the canal itself and the distal channels, but also on the trabecular meshwork and creates microperforations that can allow aqueous to flow freely through the conventional outflow system.
