Irony of Diwali - Surgical Management of Two Firecracker Injury cases

Prof. Dr. S. Natarajan, Dr Kavita Rao present two cases of ocular trauma caused by firecrackers. The first case is a 23-year-old male presented with a injury to the left eye on October 27, 2011. On examination his visual acuity in the rigth eye was 6/6 where as the left eye was hand movements close to face. Anterior segment evaluation revealed full thickness corneoscleral tear with Hyphema. We planned surgery immediately and primary corneoscleral tear repair was done. In the second case, a 15-year-old boy presented to us on the day of Diwali with history of trauma by firecracker. On examination his visual acuity in right eye was 6/6 and in left eye was appreciation of Hand movements close to face. Anterior segment evaluation revealed Traumatic mature subluxated cataractous lens with zonular dialysis and vitreous in anterior chamber. Ultrasonography of the left eye revealed moderate intensity echoes in vitreous cavity suggestive of vitreous hemorrhage with attached retina. Patient underwent 23-G Vitrectomy and lensectomy with scleral fixated IOL after 1 week. Postoperatively his visual acuity was 6/36 in the left eye. Detailed examination revealed SFIOL in place with clear media with macular subretinal fibrosis and hemorrhage which is responsible for the reduced vision.

the iol is upside down. shall i assume the iol is biconvex and makes no difference?

sir i am very very very junior, immature and naive than you but and surgeons are allowed to have different approaches but for young surgeons would you advice suturing at the limbal site first before proceeding with corneal suturing for better anatomical restoration .... i just want to clear my doubt with all hearted respect.
