David M. Dickman, MD, presents the use of the MicroPulse Laser Therapy (MLT) with the Iridex IQ 532 laser in a patient with an IOP of 16 mmHg OU. She was pre-treated with brimonidine (Alphagan P, Allergan) to prevent a pressure spike after the laser treatment. Using the laser's continuous-wave setting, he placed four spots as test burns to titrate the laser power based on the observed tissue response. Energy was delivered at a 50-µm spot and 100-ms continuous-wave exposure, starting at 500mW and increasing until a slight blanch and bubble were noted at 700mW. Sub-threshold MicroPulse treatment was then applied according to the parameters in Table 1. The he applied 120 narrowly spaced 300-µm spots to cover 360 degrees of the trabecular meshwork.
