In this video, Saad Alshamrani, MD, presents bleb transposition as a novel surgical technique designed to treat thin, leaking filtering blebs. A rectangular conjunctival flap is fashioned, the conjunctiva is first marked at the bleb site, and healthy conjunctiva is identified nasally and temporally. The distance from the limbus to the posterior edge of the bleb is carefully measured, and then doubled. This ensures adequate length for tension-free flap rotation. A conjunctival peritomy is initiated posteriorly near the fornix and extended anteriorly toward the limbus. The conjunctival flap is then rotated so that healthy conjunctiva is positioned at the limbus, while the thin, ischemic conjunctiva is relocated posteriorly toward the fornix. The flap is secured using interrupted 9-0 vicryl sutures, ensuring a watertight and tension-free closure.
