Mariano Irós, MD, describes a modification of the Yamane technique in an 85 year-old patient with a complete bag/IOL complex luxation to the vitreous cavity. If the patient has a prolene haptic lens, the author typically performs a 27G needle technique with sutureless IOL scleral fixation and external burning of the distal part of the haptic. This outer portion of the haptic usually flattens down and stays in the subconjuntival space, but in some cases the haptic may erode the conjuntiva and get exposed posing risk of endophthalmitis. In this modification, a conjunctival opening and 1.5mm scleral pocket is created where the 27G needle is inserted, so when the externalized distal end of the haptic can be tucked in the pocket.
