IOL exchange using 'Pacman' IOL cutting technique. In this case, the patient had persistent photopsias with an Acrysof acrylic toric IOL. Limbal relaxing incisions were placed to account for the corneal astigmatism, then the IOL was removed by cutting a quadrant out of the optic (creating a 'pacman' shape) and rotating the rest out of the 3 mm incision. A Tecnis acrylic monofocal IOL was inserted in the capsular bag, and postoperatively the patient's photopsias resolved.
