Tal Raviv, MD, presents the management of a 4-5+ leathery cataract. In mature cataracts, Dr. Raviv prefers a large capsulorhexis and a gentle hydrodissection. He uses a combination of horizontal and vertical chopping maneuvers to debulk the lens but it resists fracture. After viscoelevating one of the quadrants, he converts to an internal extracapsular technique.
