Arun C. Gulani, MD, presents an RK case with prior vitreoretinal surgery and macular degeneration. He uses a metal blade to keep incisions blunt. The capsule is extremely friable and the lens is mobile so management is further complicated by a poor red reflex. He explains the principles he uses to limit stress on the capsule and RK incisions.
