Jason Jones, MD, shares a case of congenital inferior iris coloboma that he repaired at the time of cataract surgery. After breaking the posterior synechiae under viscoelastic, Dr. Jones retracts the iris with hooks. Coloboma cases require extra care, because the ciliary body and zonules may be compromised. After removing the dense nucleus and implanting an IOL, Dr. Jones sutured the iris sphincter to reapproximate the pupil to a more normal position.

Dear Dr Jones, I am a sufferer of congenital coloboma, in my left eye (making it look like a cats eye/keyhole shped pupil-worse than madeline mccanns). Please could you get in touch with me? I was always told that there were no operations that could fix the pupil. It has really bothered me all of my life and i'd do anything to have normal eyes! Would it be possible to pm me please? Dear Dr. Jones, In my experience repairing the dialysis is important and can sometimes cause dysphotopsia if not addressed. I will go through some of my video's from the 90's when I developed this technique and see what I can find. I have done over 300 cases like this and have learned the hard way that any additional openings besides the pupil can be a problem. Nice job, but always room for improvement! Steve- Agree there's always room for improvement. In my more modest experience I find pts w/congenital coloboma like this accept the peripheral dialysis better than traumatic defect repair. And I always discuss that additional surgery may be needed. Haven't needed to go back yet but there will be a first. Thanks for your comments! Jason
