A patient presents 2.5 years after cataract surgery with a complaint of glare and an inability to drive at night. The haptic of an anterior chamber IOL has trapped iris in the angle and pulled the pupil superotemporally. Multiple attempts to gently extract the iris from the angle with intraocular graspers are unsuccessful. The surgeon performs a pupilloplasty with three interrupted 10–O Prolene sutures (Ethicon) to re-create a central pupil completely covered by the IOL. The knots are tightly cinched with a micrograsper intraocularly while traction on the iris is minimized.
