Tan EndoGlide DSEAK Surgery
Prof. Donald Tan presents DSAEK surgery using the Tan EndoGlide (Network Medical Product, North Yorkshire, UK) to insert the donor lenticule. A scleral tunnel approach is used to minimize induced astigmatism. The anterior and posterior lenitcules of the donor cornea are separated with a BSS cannula and transported, endothelium side up, to the donor well of the injector system. When the posterior side of the donor graft is retracted into the insertion system with forceps, the graft automatically rolls into a double coil configuration. After the tissue has naturally unrolled in the deep AC, the surgeon introduces a complete air fill to force the donor to adhere to the posterior side of the cornea.

Niclely done and very controlled procedure. I just wonder if I and an assistant could maintain the chamber with continuous air injection during descemet removal. I would think it is still easier to do this under viscoelastic and remove it after dissection.
