This is the case of a young Marfan syndrome patient with a misshapen and subluxated lens. The capsulorhexis is shaped to match the misshapen lens because the CTR will round the capsular bag and capsulotomy. Low flow and low vacuum settings are use to emulsify the relatively soft nucleus. A modified Hater CTR preloaded with gortex sutures is slide into the bag. After the fixation hook is dialed into place, Robert Cionni, MD, uses 25G retinal forceps to pull the sutures through the sclerotomy. A Toric IOL is implanted to treat corneal astigmatism.
