Intraocular Lens Implantation Without OVD

Yasar Sakarya, MD, Ugur Unsal, MD, Rabia Sakarya, MD, and Ertugrul Cuneyt Isik, MD, present four cases of IOL implantation without the use of an ophthalmic viscosurgical device. An irrigation cannula fixates the globe and inflates the bag during IOL insertion.

The IOL injector I used was Duckworth& Kent (DK7797-2)that enables one hand injecting. It is important that you should occlude the tip of cartridge with IOL to prevent anterior chamber shallowing. IOL can be implanted directly into bag. If you bent trailing haptic on to IOL while inseting the IOL into cartridge, trailing haptic will also goes into anterior chamber. In this way you can avoid trailing haptic in the incision.

We have tried your technique with excellent results, we neither use OVD to put hidrophilic lenses in the cartridge and this reduces the chance of residual OVD. Care should be taken to fully introduce the trailing haptic in the anterior chamber and don´t let it out of the eye because this may cause chamber shallowing. Thanks for sharing.

Which kind of inyector you need to use? because with alcon´s you can´t do that, you need both hands to inject the lens

Yes I use OVD to lubricate the cartridge. It si important that when placing the tip of haptic, you should occlude the lumen of cartridge with IOL. That is, Place the tip just before the IOL leaves the cartridge to prevent anterior chamber shallowing.
