thanks 4 the nice presentation but I can`t consider this technique as safe,the risky point is the pressure difference (intralenticular-AC) at the moment of puncturewhich may be increased by aspiration and sometimes pressure may be lowered ahead of the tip but not superiorly,many tech. I tried ,the best is to make small entry ,over pressurize AC with visco elastic ,small central puncture,aspirate as much fluid cortex by 26 canula re inject visco elastic by that time ant.capsule is excavated and you can do the main incision and do your Rx safly
what is the setting of power, flow rate, and Vacuum.
The allegation that "no complications are possible" is probably an overstatement...When entering the ant.capsule do you go down with foot pedal on 3(US),and with what vacuum?

If you phaco right through the capsule with the port facing DOWN, you create a round hole that resist tearing while you decompress the capsular bag.
tried this once and the tear went right out to the zonules the moment the phaco needle buzzed the capsule. but maybe ill try it again
