Combined Phaco-Vitrectomy with 25g Valved Cannulae

Rehman Siddiqui, MD, highlights the use of 25-gauge Edgeplus valved cannula system (Alcon) in a combined phaco-vitrectomy case. The patient was a 60-year-old diabetic with dense vitreous hemorrhage and NS+3 cataract. Note how there was no fluid egress from the valved cannulas during phacoemulsification part of the procedure, when the bottle height was at maximum. On the other hand there was minimal resistance to thin, flexible 25g laser probe.

Great case. Thanks for sharing. Thankfully, a low brow allowed superior approach for the phaco. In many cases, doing the combined procedure, including the vitrectomy, both from the temporal aspect, is a nice way to approach (accepted RETINA - "A Temporal Approach for Small-Gauge Pars Plana Vitrectomy Combined with Anterior Segment Surgery" Tarantola, Graff, Somali, Mahajan
