Refractive Surgery with Corneal Scars Secondary to HSV Keratitis

Herb Kaufman, MD, and Richard Awdeh, MD, discuss the use of antiviral medications during the peri- and postoperative period in refractive laser patients with stromal scars to inhibit the activation of corneal herpes. According to Dr. Kaufman, the use a topical antiviral with a lower toxicity profile that targets infected cells only, like ganciclovir, would be preferred since it would be less likely to interfere with wound healing. Finally, the doctors talk about preferred strategies for minimizing the visual impact of old herpetic scars, including penetrating keratoplasty, manual and femtosecond lamellar dissection.
