Phaco with a Double-sutured, Intraoperatively modified CTR in Microspherophakia

This is the case of a very small eye: a 10-mm white-to-white microspherophakic eye of a 37-year-old Caucasian man. DisCoVisc OVD (Alcon, Fort Worth, TX) is instilled to pressurize the AC and partially flatten the highly convex anterior surface of the cataractous lens to produce a flatter anterior capsule. Trypan blue is instilled to help visualization of the capsulorhexis. Because of the weak zonules, the nucleus was extracted under generous amounts of OVD and a double-fixated CTR is sutured to both sides of the bag with a bimanual approach.
