I have been performing the polishing of Posterior surface of anterior capsule the last four years. I use it in all cases soft low Vacuum with bimanual textured cannulas I/A (From Alcon) for better shaving effect of visible Lens Epithelial Cells. I perform that only in cases of competent Zonula System .This was my standard technique even in combined cataract and 23 G Micro Incision Vitrectomy Surgery , The polishing of the underside of the anterior capsule could always enhance the visualization of the peripheral retina. Our observations showed lower rates of PCO with at least more than one year follow up. We did not make any study for that. We did not observe any case of capsule contractions or phimosis.

I used to routinely and thoroughly polish both PC and underside of AC, but after listening to both Howard Fine, Lisa Arbisser and others on the subject, I have removed this step (AC polishing). Comments?
