I recently used a CTR immediately after hydrodilineation prior to phaco in a case with a rock-hard nucleus and about 50% zonular dehiscence. It worked great, stabilizing the eye allowing for safer phaco. If I trapped some cortex it did not cause any post-op inflammation.
