Sulcus Fixation of an IOL Using Scleral Tunnels

Drs. Som Prasad and Gabor Scharioth present a strategy to achieve secure sutureless intrascleral haptic fixation. A standard three-piece IOL with a haptic size of 13.5 mm is inserted into the anterior chamber, the haptics are externalized and secured within the scleral tunnel to support the lens in the posterior chamber.

may i ask,,after put the haptis in the scleral,, how do you keep haptics in the place,,? do you need to suture the horizontal tunnel you made ? or just close the conjunctiva,,?

Zico, no haven't had endophthalmitis or hypotony. Our medium term follw up was published in JCRS - February 2010 (Vol. 36, Issue 2, Pages 254-259), this may be of interest to you.

Som Prasad (23 months ago)

excellent technique, have you experienced hypotony or endophthalmitis?

Dr Mario, yes you are right. This technique works best with a 3 piece IOL. Best Wihses, Som

Som Prasad (24 months ago)

Excelent technique but could not do it with a single piece LIO, keeps breaking.

Thanks for the comments.

Excellent idea . I tried that already in four patients. This is a very promising technique . I feel more secure with it than with suturing the IOL.But it needs a learning curve. .

EXCELLENT VIDEO, BUT IN CASES WITH NO IRIS DEFECTS A IRIS SUTURED IOL IS ALSO A GOOD OPTION AND HAS NO POTENTIAL COMPLICATIONS RELATED TO THE IOL'S HAPTICS CONNECTING THE INTRA-EXTRAOCULAR ENVIRONMENTS
