A punctured posterior capsule
After an epinucleus fragment was seen floating behind the posterior capsule, it was evident that capsule was punctured. The use of microincisional biaxial phaco allowed the surgeon to keep the irrigator far away from the posterior capsule while safely removing the nucleus.
