Richard Mackool, MD, marks the steep axis to identify the location of the penetrating limbal relaxing incisions. Upon injection of viscoelastic, the iris is noted to be floppy. Hydrodissection is performed with a broad flat cannula and the nucleus is removed using high vacuum.
