I used to do it that way with an ant vit but found it was very difficult to get the "huge" lens in the eye.  This way I can get the lens in and know where it is.  With an open capsulotomy, if I can't get it in the bag with confidence I have to take the lens out and put in a 3 piece so the haptics can be in the sulcus if necessary.
Leaving the bag intact and doing a PPV has made it possible for me to put lenses in younger and younger patients with confidence that the lens is in the bag.  I don't really think the posterior pressure is the issue.  It is the small bag and the big lens.   I am considering using the Softec lens on my next baby eye as it is 1 mm shorter.
